Referral form for individual exercise physiology services

If you have a health condition or injury you may be eligible for up to 5 Medicare rebates for exercise physiology services/year. One referral is equal to a $58 rebate. That means you only need to pay a gap fee.

Follow these easy steps:

1. Download the “Referral form for individual allied health services below” and take this to your GP.
2. Your GP will determine if you are eligible.
3. Once you have your referral, give us a call to book your initial exercise physiology consultation.

This form can also be used for online services.

 

Referral form for Group Allied Health Services under Medicare for patients with type 2 diabetes

If you have Type 2 Diabetes, in addition to above, you may also be eligible for 1 initial consultation + 8 group exercise sessions. 

Follow these steps:

1. Download the “Referral for group allied health services below” and take this to your GP.
2. Your GP will determine if you are eligible.
3. Once you have your referral, give us a call to book your initial exercise physiology consultation.

Private health funds

Exercise physiologist are recognised as an allied health professional. You may be eligible for rebates for exercise physiology consultations and training sessions. Check with your health fund.
